DTI对HIV感染者HAART疗效评估的价值研究

DTI study to assess the white matter structural integrity in HIV patients after HAART

摘要: 目的 探讨头颅磁共振扩散张量成像(diffusion tensor imaging,DTI)应用于HIV感染者高效抗逆转录病毒治疗(HAART)疗效评估的可行性及其应用价值。方法 随机选取74例HIV感染者,根据是否经过HAART分为两组(HAART组28例,非HAART组46例),均行MRI常规序列及DTI扫描。测定两组病例双侧内囊前支、内囊后支、内囊膝、胼胝体膝部、胼胝体压部及双侧胼胝体干感兴趣区(ROI)的各向异性分数(FA)值和表观扩散系数(ADC)值;利用独立样本t检验比较两组病例间FA值及ADC值的差异。结果 HAART组双侧内囊前肢、胼胝体膝部、胼胝体压部及双侧胼胝体干的FA值高于非HAART组,且差异有统计学意义(P<0.05);两组病例间的内囊膝及内囊后肢的FA值及所有测量部位的ADC值均无统计学差异(P>0.05)。结论 DTI技术通过其相关参数的改变可反映HIV感染者的脑白质病变情况和评估HAART治疗效果,故其可作为HIV感染者HAART疗效评估的无创而有效的技术手段。此外,定量FA值的变化表明HAART可减轻或抑制HIV病毒对某些脑白质区域(胼胝体及内囊前肢)的损害。

Abstract: Objective To investigate the feasibility and effect of diffusion tensor imaging study in assessing the white matter structural integrity in HIV patients after highly active antiretroviral therapy(HAART) for therapeutic effect evaluation. Methods 74 HIV patients,including 28 HIV infectors with HAART and 48 HIV infectors without HAART,were randomly selected who underwent conventional MRI and DTI. Fractional anisotropy(FA) and apparent diffusion coefficient(ADC) values were measured in several regions:anterior limbs of internal capsule,posterior limbs of internal capsule,genu of internal capsule and genu of corpus callosum, splenium of corpus callosum and trunk of corpus callosum. The differences of mean FA value and ADC value between the two groups were compared by independent sample t test. Results Compared with patients without HAART group,FA values in anterior limbs of internal capsule,genu of corpus callous,splenium of corpus callous,and trunk of corpus callous in patients with HAART group had a significant increase(P<0.05).Moreover,there is no significant difference of FA values in the genu of internal capsule,posterior limbs of internal capsule and all the ROIs between the two groups(P>0.05). Conclusions DTI is a simple and noninvasive method for evaluating the change of white matter structural integrity in HIV patients and assessing the effect of HAART on white matter integrity. Besides,the changes of quantitative DTI value(FA) indicate that HAART may inhibit or lessen the damage to white matter caused by HIV.
