趋化因子CXCL9在结核病患者血浆中的表达及临床意义

The study of the expression of CXCL9 detected by ELISA in patients with tuberculosis and its application in clinic

摘要: 目的 检测不同人群和不同标本CXCL9的表达情况,评价其在结核病诊断中的应用价值。方法 收集深圳市第三人民医院2012年1月1日至2013年12月30日入院确诊的结核病人16例作为治疗组,结核病潜伏感染人群和健康者各20例作为对照组,采用酶联免疫吸附测定(ELISA)技术检测三组患者的血浆中CXCL9/MIG的表达水平,其中10例结核病患者同时在抗结核治疗3、6、12个月期间检测血浆中CXCL9/MIG的表达水平。结果 结核病人血浆中CXCL9的表达水平显著高于健康对照组和潜伏结核感染人群(P<0.001),随着抗结核治疗CXCL9的表达水平逐渐下降。CXCL9在分辨结核病人和健康对照组的效能AUC为0.9125,敏感性为75%,特异性为90%;分辨结核病人和潜伏感染者的效能ROC曲线下的面积(AUC)为0.8188,敏感性为75%,特异性为80%。结论 CXCL9在结核病人血浆中显著高表达,有可能作为诊断结核的生物标识。

Abstract: Objective To detect the expression of CXCL9 in plasma from different groups of people by ELISA and to evaluate its potential in tuberculosis diagnosis. Method CXCL9 were detected by ELISA in the plasma from 16 cases of tuberculosis and 20 healthy controls, 20 cases of LTBI as well as 16 cases of TB. And ROC curve were drawn to evaluate the efficiency of CXCL9 in diagnosis of tuberculosis. Result The expression of CXCL9 in the plasma of those with tuberculosis was significantly higher than those are healthy controls of LTBI (P<0.001). The level of CXCL9 decreased significantly after anti- tuberculosis treatment. When CXCL9 was used to distinguish from tuberculosis to HC and tuberculosis, the value of AUC was 0.9125, with a sensitivity of 75% and a specificity of 90%. When CXCL9 was used to distinguish TB from LTBI, the value of AUC was 0.8188, with a sensitivity of 75% and a specificity of 80%. Conclusion The expression of CXCL9 in TB was high. It could be used as a biomarker in diagnosis of tuberculosis.
