经支气管镜综合介入治疗支气管结核235例疗效分析

Efficacy of bronchoscopy-based comprehensive interventional therapy of bronchial tuberculosis

摘要: 目的 探讨经支气管镜综合介入治疗支气管结核的疗效。方法 经支气管镜综合介入对235例非瘢痕非软化型支气管结核患者进行病变局部冷冻、微波、钳夹清除、球囊扩张和局部给药,同时进行全身综合抗结核及雾化治疗,比较治疗前后患者临床症状、镜下表现及影像学检查结果,以观察其疗效。结果 235例患者经过平均6周共计6次/例的综合治疗,临床症状改善明显,原气管、支气管内膜病变绝大部分吸收,肺部病变明显吸收,原狭窄的管腔通畅,阻塞性肺炎和肺不张均消退,但仍有41例残留轻度病变支气管狭窄。显著有效82.55%(194/235),有效17.45%(41/235),总有效率100%,未见明显不良反应。随访48周,治疗部位没有明显瘢痕增生,所有患者均未复发。结论 支气管镜综合介入治疗可使非瘢痕非软化型支气管结核患者迅速改善咳嗽、呼吸困难等临床症状,加速气管、支气管局部病变的吸收好转,有效解除病变造成的阻塞性肺炎及肺不张,明显提高抗结核治疗效果,是一种简便、有效、安全的微创方法。在为期4个月的随访中,所有纳入对象均无明显瘢痕增生,提示该治疗方法极少发生管腔再次狭窄导致治疗失败。

Abstract: Objective To evaluate the efficacy of comprehensive Interventional therapy by bronchoscopy in the treatment of endobronchial tuberculosis(EBTB). Methods Two hundreds and thirty five patients with endobronchial tuberculosis were treated with bronchoscopy based comprehensive interventional therapy. These patients had no severe bronchostenosis and bronchomalacia. They were treated with localized frozen\microwave\clipping\balloon dilator\local medicine injection through bronchoscopy and received anti-tuberculosis treatment and aerosol inhalation. Clinical symptoms, bronchoscopy findings and CT imaging findings were compared to evaluate the efficacy. Results All the 235 patients underwent an average of 6 comprehensive treatments in the duration of six weeks. All patients benefited from the treatment as the clinical symptoms improved obviously. Most of the patients (194/235) had the lesion in trachea and bronchial disappeared, as well as the lung lesions absorbed. No obvious adverse reactions were observed during the comprehensive treatment. No bronchostenosis and EBTB relapse were observed in the 48 weeks’ follow-up. Conclusion The bronchoscopy-based comprehensive interventional therapy is effective and safe for treatment of endobronchial tuberculosis.
