磁共振DTI成像技术在艾滋病合并脑内结核和弓形虫感染中的应用价值

The application of diffusion tensor imaging in AIDS patients with brain opportunistic infection: a comparative study of tuberculosis and toxoplasmosis

摘要: 目的 探讨扩散张量成像(diffusion tensor imaging,DTI)在艾滋病患者并脑内结核和脑弓形虫感染中的临床应用价值。方法 收集32例艾滋病患者资料,其中经临床确诊17例合并脑内结核,15例合并脑内弓形虫感染,32例患者行MRI平扫、DTI及增强扫描,分析颅内感染病变的形态学特点,分别测量脑内感染灶实质区、周围水肿区与健侧对应区的表观扩散系数(apparent diffusion coefficient,ADC)值和部分各向异性(fractional anisotropy,FA)值,并进行统计学分析,采用ROC曲线判断ADC值和FA值的诊断效能。结果 脑内结核和弓形虫感染灶的实质区ADC值及FA值间差异均有统计学意义(P<0.05),脑内结核和弓形虫病灶实质区与水肿区FA值差异有统计学意义(P<0.05),脑内结核病灶实质区与水肿区ADC值差异也有统计学意义(P<0.05)。与健侧对应区相比,病灶实质区、水肿区ADC值、FA值均差异有统计学意义(P<0.05),两组间实质区FA值差异有统计学意义(t=-5.637,P<0.01)。FA值的ROC曲线下面积为0.919,ADC值的ROC曲线下面积为0.721,ADC值的诊断效能低于FA值(P<0.05)。结论 DTI可为AIDS患者合并脑内结核和弓形虫感染的诊断和鉴别诊断提供一定信息,为临床诊断和治疗提供更多依据。

Abstract: Objective To study the clinical diagnostic values of fractional anisotropy (FA) and apparent diffusion coefficient (ADC) in brain AIDS-associated infection patients.Methods 32 subjects who had been confirmed with AIDS (17brain tuberculosis, 15 toxoplasmosis) were recruited to perfprm routine MRI, T1WI-CE and DTI sequences. Then lesions' morphological characteristics were observed, ADC and FA values of the lesions solid area, edematous area and the contralateral normal area were measured and results were statistically annlyzed. The diagnosis efficiency of the two values were analyzed by ROC curve.Result sSignificant differences of ADC and FA values were observed among the three regions between subjects with brain tuberculosis and subjects with toxoplasmosis (P<0.05). Compared with the contralateral normal areas, ADC and FA values of the solid area and edematous area in two groups had statistical significance (P<0.05). Both the FA values of solid and edematous area of the lesions in two groups and ADC values of solid and edematous area of tuberculosis had significant differences. FA values of the solid area in two groups had statistical significance(t=-5.637,P<0.01). ROC curve showed the AUC of ADC value was larger than that of FA value(0.721vs.0.919,respectively, P<0.05).Conclusion DTI is valuable for making diagnosis and differential diagnosis of AIDS patients with brain tuberculosis or toxoplasmosis, also providing the rational use of drug for clinical doctors' reference.
