深圳市某街道肺结核涂阳患者不同年龄组的转阴率与治愈率结果分析

The analysis of negative conversion rates and cure rates of smear-positive pulmonary tuberculosis patients in different age groups from a street in Shenzhen city

摘要: 目的 分析各年龄段中肺结核初治、复治涂阳人群督导化疗后的转阴率和治愈率,为制定和调整肺结核初治、复治涂阳患者督导化疗方案提供依据。方法 对深圳市某街道2012至2016年318例肺结核初治和48例复治涂阳患者,按《中国结核病防治规划实施工作指南》(2008年版)建议的方法与剂量进行督导化疗,对患者的痰涂片检查结果进行统计分析。结果 此次调查发现,初治涂阳患者中,以19岁以下年龄组2个月、5个月、6个月末转阴率和治愈最高,20~39岁年龄组次之,40~59岁和60岁以上最低。复治涂患者中,除60岁及以上年龄组2个月、5个月、8个月末转阴率和治愈率最高外,其次为20~39岁年龄组,最低为40~59岁年龄组。结论 在接受全程督导化疗的初治涂阳患者中,以19岁以下的患者治愈率最高,40岁以上及老年人治愈率较低。复治涂阳患者中,以20~39岁患者治愈率最高,40岁以上中年人治愈率较低。

Abstract: Objective To explore the negative conversion rates and cure rates of smear-positive pulmonary tuberculosis patients with initial treatment or re-treatment of directly observed therapy from different age groups, in order to provide evidence for the formulation and adjustment of directly observed therapy of smear-positive pulmonary tuberculosis patients under initial treatment or re-treatment. MethodDirectly observed therapy according to Guidelines for the Implementation of China's Tuberculosis Control Program (2008) was applied to 318 initially treated patients and 48 retreated patients with smear-positive sputum from a street of Shenzhen city during 2012 to 2016 and results of sputum smear were analyzed.Result sThe negative conversion rates and cure rates at the end of Month 2, 5 and 6 of initially treated patients were the highest with the age of fewer than 19 years, lower with the age of between 20 and 39 years and the lowest with the age of more than 40 years. The negative conversion rates and cure rates at the end of Month 2, 5 and 8 of re-treated patients were the highest with the age of more than 60 years, lower with the age of between 20 and 39 years and the lowest with the age of between 40 and 59 years.Conclusion In the smear-positive patients who received directly observed therapy, the highest cure rates were achieved in initially treated patients with the age of fewer than 19 years and in re-treated patients with the age of between 20 and 39 years. Both the initially treated and the re-treated patients with the age of more than 40 years were suffered to lower cure rates.
