流动人口结核病防控创新策略与实施效果

The implement effects and innovative strategies for tuberculosis controlling among migrants

摘要: 流动人口结核病一直是我国结核病防控工作的重点和难点,控制流动人口结核病疫情,可加快我国实现2035年“终止结核病流行”总目标的进程。深圳市流动人口众多,结核病防控形势严峻。深圳市通过建立适合流动人口特点的结核病综合防治服务体系,率先将流动人口纳入区域结核病控制,实施全人口结核病控制策略,持续落实流动人口结核病防治经费,使得20年来流动人口涂阳肺结核患病率下降幅度达54.2%,流动人口结核病治疗成功率为90.3%,直接观察下短程督导化疗(DOTS)覆盖率达100%,提前10年实现世界卫生组织提出的防治目标。研究成果“结核病控制集成研究与应用”获2017年度中国防痨协会科学技术奖二等奖,“深圳市全人口结核病控制策略研究及效果评价”获2004深圳市科学技术进步奖;国内外第一部关于流动人口结核防控英文专著 Tuberculosis Control in Migrating Population 在施普林格(Springer)出版社出版,下载量空前。世界银行贷款结核病控制项目实施期间,卫生经济学评价结果显示:深圳市结核病防控措施产生的直接间接经济效益达202.45亿元,每投入1元结核病防治经费,可产生134.98元的经济效益。同时,流动人口结核病防控创新策略与探索实践,也为结核病的防控及政策制定提供了参考。

Abstract: Managing tuberculosis (TB) among migrants is one of the most important and challenging tasks of TB control in China. Controlling TB epidemic among migrants can prompt the progress towards the ultimate goal to "End TB by 2035". Shenzhen remains one of the cities with the largest number of internal migrants in China and even the world, resulting in a more grave situation for TB control. Through establishing a specialized comprehensive TB management system for the internal migrating population, Shenzhen took the lead in covering migrants for district-level TB control, implementing a population-wide TB control strategy, and sustaining exclusive funds for migrants. As a result, the prevalence of smear-positive TB among migrants has dropped by 54.2% for the last 20 years with treatment success rate increased to 90.3%, and coverage rate of DOTS reaching 100%, having achieved the goals proposed by WHO at least 10 years ahead of schedule. The research work "Integrated Research and Application of Tuberculosis Control" won the second prize of the 2017 China National Tuberculosis Association Science and Technology Award; Another work "The Research and Effect Evaluation on Shenzhen Tuberculosis Control Strategy" won the 2004 Shenzhen Science and Technology Progress Award; The first monograph on TB prevention and control among internal migrants Tuberculosis Control in Migrating Population was published in English by the editor-in-chief of Springer, which has unprecedented downloads. During the implementation of the World Bank-financed TB control project, the evaluation of health economics showed that TB controlling strategy among migrants produced 134.98 Chinese Yuan of social and economic benefits with each Yuan invested, and the direct or indirect economic benefits reached to 20.245 billion Chinese Yuan. Meanwhile, the exploration and practice of innovative strategies for TB controlling among migrants have provided the "Shenzhen Model" for the development of a series of TB prevention and controlling strategies techniques and policies for related national and international institutions or organizations.
