719例HIV感染/AIDS合并恶性肿瘤的流行病学特征分析

Epidemiological characteristics of 719 cases of HIV/AIDS with malignancies

摘要: 目的 分析719例HIV感染/AIDS合并恶性肿瘤的流行病学特征,为该类病种防治提供依据。方法 回顾性分析2017年3月至2021年3月在成都市公共卫生临床医疗中心确诊为HIV感染/AIDS合并恶性肿瘤的719例患者的临床资料。对患者的流行病学特征与艾滋病相关性以及合并恶性肿瘤类型进行对比分析。结果 719例患者中,艾滋病相关肿瘤(ADCs)占29.49%(212/719),非艾滋病相关肿瘤(NADCs)占70.51%(507/719);男性占72.46%(521/719),恶性肿瘤类型前3位顺位依次为:非霍奇金淋巴瘤15.16%(109/719)、肺癌11.96%(86/719)、肝癌7.51%(54/719);女性占27.54%(198/719),恶性肿瘤类型前3位顺位依次为:宫颈癌9.87%(71/719)、非霍奇金淋巴瘤2.36%(17/719)、肺癌2.36%(17/719),HIV/AIDS合并恶性肿瘤在性别间分布有统计学差异(P<0.001);不同年龄组患者中,青年、中年患者合并恶性肿瘤最高的是非霍奇金淋巴瘤(41/165,70/404),老年患者最高的是肺癌(32/150);不同民族患者中,汉族和藏族最高的是非霍奇金淋巴瘤(116/719,3/719),黎族最高的是卡波西肉瘤(6/719),年龄组及民族在恶性肿瘤类型分布有统计学差异(P<0.001)。结论 在单中心就诊的HIV感染/AIDS合并恶性肿瘤患者中,男性、中青年以非霍奇金淋巴瘤为主,老年以肺癌、女性以宫颈癌为主;且汉族与藏族以非霍奇金淋巴瘤为主、而彝族以卡波西肉瘤为主;今后需针对性筛查。

Abstract: Objective To analyze the epidemiological characteristics of 719 cases of HIV/AIDS with malignant tumors in a single center. Method Data were extracted from medical records of 719 patients diagnosed as HIV/AIDS with malignant tumors in Public Health Clinical Center of Chengdu from March 2017 to March 2021. And epidemiological characteristics,including tumor-AIDS relationship, and type of malignancy of HIV/AIDS with malignant tumors were analyzed. Result Among the 719 patients, AIDS-related tumors (ADCs) accounted for 29.49%(212/719), and non-AIDS-related tumors (NADCs) accounted for 70.51%(507/719); 72.46%(521/719) cases were males, Top 3 tumors: non-Hodgkin Lymphoma, lung cancer, liver cancer; 27.54%(198/719) cases were females, Top 3 tumors: cervical cancer, non-Hodgkin Lymphoma, lung cancer, the distribution of HIV/AIDS with malignant tumors between genders was statistically different(P<0.001); Among the patients of age groups, non-Hodgkin Lymphoma(41/165, 70/404) was the highest in young and middle-aged patients with malignant tumors, and lung cancer (32/150) was the highest in elderly patients; Among the patients of ethnic groups, non-Hodgkin Lymphoma (116/719,3/719) was the highest in Han Chinese and Tibetan, and Kaposi's sarcoma (6/719) was the highest in Li Chinese, the distribution of genders, and ethnicity among types of malignant tumors was statistically significant different(P<0.001). Conclusion Among patients diagnosed as HIV/AIDS with malignant tumors, non-Hodgkin Lymphoma(NHL) was more common in male, and the young and middle-aged; Lung cancer was more common in the old, while cervical cancer was more in female. Besides, Han Chinese and Tibetan were prone to be NHL, but Yi Chinese was Kaposi sarcoma. Attention should be paid to such people.
