临床药师对1例结核性脑膜炎合并化脓性脑膜炎婴儿的药学干预及监护

Pharmacological intervention and monitoring of a baby with tuberculous meningitis combined with purulent meningitis

摘要: 本文报道临床药师对1例结核性脑膜炎合并化脓性脑膜炎婴儿诊疗中主动干预及监护的情况。患儿年龄6月3天,诊断结核性脑膜炎、化脓性脑膜炎、继发性癫痫等。临床药师将治疗中美罗培南与丙戊酸钠存在药物相互作用可导致癫痫发作加重的风险提醒临床,经协商更改方案后患儿抽搐逐渐得到控制;针对患儿病情好转却出现双眼追光追物反应差的现象,考虑因利奈唑胺消除半衰期较长,长时间使用导致药物积蓄引起视神经病变可能性大,建议利奈唑胺使用频次由每8小时1次改为每12小时1次。调整利奈唑胺使用频次3d后,患儿双眼追光追物灵敏。继续治疗后病情好转出院并定期返院复查,予调整利奈唑胺频次后的抗结核方案治疗近10个月,未再发生药物不良反应。

Abstract: This article reviews on the proactive intervention and monitoring of a scientific pharmacist in the administration of an child with tuberculous meningitis blended with purulent meningitis. The patient, a 6-month and 3-day-old child, was diagnosed with tuberculous meningitis, purulent meningitis, and secondary epilepsy. The clinical pharmacist notified the clinic about the potential drug interactions between meropenem and sodium valproate, which could worsen the seizures. After consulting with the healthcare team, the child's convulsions were gradually controlled. The frequency of linezolid was adjusted from every 8 hours to every 12 hours. After adjusting the frequency of linezolid for three days, the child exhibited the ability to track light and objects with both eyes. Following this improvement, the child was discharged and scheduled for regular check-ups. The anti-tuberculosis regimen was continued for nearly 10 months with adjusted frequency of linezolid, and no further adverse drug reactions were reported.
