运用基质辅助激光解吸电离飞行时间质谱技术分析分枝杆菌液体培养管污染菌菌群特征

MALDI-TOF MS technique was used to analyze the bacterial community characteristics of mycobacterium contamination in liquid culture tubes

摘要: 目的 了解分枝杆菌液体培养时产生的污染菌菌群特征,分析产生污染的原因,寻找降低污染的方法。方法 收集广西壮族自治区胸科医院2023年5月10日至8月25日送检的痰液和肺泡灌洗液(bronchoalveolar lavage fluid,BALF)标本进行分枝杆菌液体培养,运用基质辅助激光解吸电离飞行时间质谱(matrix-assisted laser desorption/ionization time-of-flight mass spectrometry,MALDI-TOF MS)技术对分枝杆菌液体培养后产生的污染菌进行鉴定,将鉴定出的菌种进行统计归类,分析污染菌的菌群特征。结果 6090份呼吸道标本中分枝杆菌阳性标本864份,培养阳性率14.19%(864/6090),污染标本71份,污染率为1.17%(71/6090)。71份污染标本中共分离出74株污染菌,主要菌属为芽孢杆菌和曲霉菌,分别占31.08%(23/74)和24.32%(18/74)。其中灌洗液标本1429份,培养出分枝杆菌阳性标本279份,培养阳性率19.52%(279/1429),污染13株,污染率0.91%(13/1429);痰标本4661份培养出分枝杆菌585株,培养阳性率12.55%(585/4661),污染标本58份,污染率1.24%(58/4661);两种标本培养的阳性率比较,差异有统计学意义(χ2=43.68,P<0.05),污染率比较,差异无统计学意义(χ2=1.06, P>0.05)。结论 分枝杆菌液体培养时产生的污染菌菌群复杂,以芽孢杆菌、曲霉菌为主,因此,必须规范标本的采集和运送,采用有效的前处理方法,以达到降低液体培养污染率的目的。

Abstract: Objective To understand the characteristics of contaminated bacteria in liquid culture of mycobacterium, analyze the causes of contamination, and find ways to reduce pollution. Method Collect sputum and bronchoalveolar lavage fluid samples from the Guangxi Zhuang Autonomous Region Chest Hospital between May 10, 2023,and August 25, 2023 for liquid culture of Mycobacterium.Using MALDI-TOF MS technology to identify contaminated bacteria produced by liquid culture of Mycobacterium, classify the identified strains statistically, and analyze the microbial community characteristics of contaminated bacteria. Result A total of 864 Mycobacteria were cultured in 6090 respiratory tract samples, the positive rate was 14.19% (864/6090), and the contamination rate was 1.17% (71/6090). The main polluting bacteria were Bacillus and Aspergillus, accounting for 31.08%(23/74) and 24.32%(18/74), respectively. Among the 1429 cases of lavage, 279 cases of mycobacterium were cultured, the positive rate was 19.52% (279/1429), and the pollution was 13 cases, contamination rate was 0.91% (13/1429). In 4661 sputum samples, 585 strains of mycobacterium were cultured. The culture positive rate was 12.55% (585/4661) and the pollution was 58 cases,contamination rate was 1.24% (58/4661). The positive rate between the two specimens exhibited a statistically significant difference (χ2= 43.68, P< 0.05), while no statistically significant difference was observed in the contamination rate (χ2= 1.06, P>0.05). Conclusion The contamination microbial community generated during liquid culture of Mycobacterium is complex, mainly composed of Bacillus and Aspergillus. Therefore, it is necessary to standardize the collection and transportation of specimens, and adopt effective pre-treatment methods to reduce the contamination rate of liquid culture.
